ABSTRACT

Objective:To analyze the predictors of poor outcomes after emergency intracerebral thrombectomy based on the characteristics of cerebral angiography. Methods:A total of 146 patients with acute ischemic stroke (AIS) who received endovascular treatment in Loudi Central Hospital from March 2019 to February 2022 were included in the study, and digital subtraction angiography (DSA) was performed on the patients. The patients were divided into a good prognosis group (95 cases) and a poor prognosis group (51 cases) by the modified Rankin scale 3 months after operation. Gender, age, time from onset to visit, time from onset to puncture, proportion of intravenous thrombolysis, occlusion site, treatment strategy, National Institute of Health Stroke Scale (NIHSS) score, core infarct volume, ischemic hypoperfusion volume, collateral circulation classification, and venous drainage status were compared between the two groups score; Logistic regression was used to analyze the risk factors affecting the poor prognosis of patients; Receive Operating Characteristic (ROC) curve was used to analyze the predictive value of collateral circulation classification and venous drainage status score for poor prognosis of patients, and the differences in general data and imaging data were compared between groups with different collateral circulation grades and venous drainage status. Results:Compared with the good outcome group, the time from onset to visit, NIHSS score, core infarct volume, ischemic hypoperfusion volume, the proportion of thrombectomy alone, and collateral circulation classification in the poor outcome group [2 (2, 3) levels. 2 (1, 2) level] and venous drainage score [5 (4, 6) points vs. 6 (6, 8) points] increased ( P<0.05), and the proportion of recanalization grade 2b/3 decreased ( P<0.05); NIHSS score, collateral circulation grade and venous drainage status were predictors of poor outcome within 3 months after mechanical thrombectomy ( OR = 2.51, 1.93, 2.61, P<0.05); collateral circulation grade and venous drainage score predicted mechanical thrombectomy in patients with AIS, the area under curve (AUC) of poor outcome after thrombectomy were 0.714 and 0.829, respectively; the time from onset to visit between patients with poor collateral circulation, moderate and good AIS [(236.95 ± 21.03) min, (250.41 ± 21.32) min, (255.72 ± 20.98 min)], core infarct volume [52 (17, 80) ml, 25 (15.5, 30) ml, 15 (10, 25) ml] and venous drainage scores [5 (4, 6) points, 5 (5, 8) points, 5 (5, 8) points] were significantly different ( P<0.05); time from onset to visit in patients with poor venous drainage, moderate and good AIS (234.81 ± 21.22 min), (256.83 ± 20.88) min, (258.97 ± 21.35) min], core infarct volume [17(13, 45) ml, 26(25, 29) ml, 20 (11, 29) ml] and collateral circulation classification [2 (1, 2) level, 2 (1, 3) level, 2 (2, 3) level] were significantly different ( P<0.05). Conclusions:Collateral grading and venous drainage scores based on DSA imaging were predictors of poor outcomes within 3 months of mechanical arterial thrombectomy in patients with AIS.

ABSTRACT

Objective:To explore the value of karyotyping and chromosomal microarray analysis (CMA) in the prenatal diagnosis of balanced translocation/inversion carriers. Methods:This was a retrospective study involving 117 balanced translocation/inversion carrier couples. Among them, 90 women had a history of spontaneous abortion(≥2 times), stillbirth, fetal multiple malformations, or giving birth to children with chromosome abnormality disease and the peripheral blood karyotyping and fluorescence in situ hybridization testing confirmed that one partner was balanced translocation/inversion carrier. The present pregnancies of these cases were spontaneous and lasted until 18-25 weeks. The other 27 cases were confirmed by chromosome examination at the present pregnancy after the indication of fetal structural abnormalities by fetal karyotyping due to advanced maternal age and abnormal ultrasound and prenatal screening results. The results of karyotyping and CMA by amniocentesis during 18 to 25 gestational weeks were all summarized and described. Results:The successful rate of both methods was 100.0% (117/117). Unbalanced and balanced translocation/inversion were detected in seven (6.0%) and 39 (33.3%) fetuses by karyotyping, respectively. CMA revealed 14 fetuses with pathogenic copy number variation (CNV) and one with variants of uncertain significance(VUS), with an anomaly detection rate of 12.8% (15/117). Among the 15 cases with CNV, 13 were related to the parental translocation/inversion, one with de novo mutation (22q11.2 microdeletion syndrome), and one Duchenne muscular dystrophy mutation carrier. Based on the results of karyotype and CMA, there were 12 fetuses with unbalanced chromosomal fragments (10.3%), 37 fetuses with balanced translocation/inversion (31.6%), and 68 fetuses with normal chromosomes (58.1%). Conclusions:The combination of karyotyping and CMA can provide more accurate prenatal genetic diagnosis when one of a couple carries balanced chromosomal translocations/inversion.

ABSTRACT

Objective:To study the correlation between the acute-phase characteristics of motor evoked potential (MEP) and severities of spinal cord injury in patients with acute cervical hyperextension injury and central cord syndrome (CCS).Methods:Retrospectively analyzed were the data of 45 patients with acute cervical hyperextension injury and CCS (observation group) who had been admitted to Department of Orthopedics, Tongji Hospital Affiliated to Tongji University from December 2018 to July 2021 and 20 healthy controls. Examination of transcranial magnetic stimulation-induced MEP was performed in patients with CCS and healthy controls using a magpro x100 magnetic stimulator, and recording was conducted in bilateral abductor pollicis brevis (APB). The characteristics of MEP waveform latency, amplitude and motor threshold were described and compared between the healthy control and observation groups; the correlations were analyzed between the MEP latency and the severity of spinal cord injury [American Spinal Injury Association (ASIA) total score and motor function of Upper Extremity Motor Subscores (UEMS)] in the observation group. According to different MEP-induced states, the patients in the observation group were divided into a resting group ( n=19), a facilitation group ( n=18), and a no-waveform group ( n=8). The severity of spinal cord injury (ASIA total score) and the functional independence of the spinal cord (SCIM-Ⅲ score) were compared among the 3 groups to analyze the correlation between the MEP-induced state and the severity of spinal cord injury (ASIA total score). Results:The observation group had a significantly longer MEP latency [(30.16±6.32) ms], a significantly smaller amplitude [(0.54±0.30) mV] and a significantly higher motor threshold [(65%±11%)] than the healthy control group (all P<0.05). The MEP latency in the observation group was significantly correlated with ASIA total score ( r=-0.730, P<0.001) and UEMS ( r=-0.740, P<0.001). The ASIA total score and SCIM-Ⅲ score were significantly different among the 3 groups ( P<0.05), and the MEP-induced state was significantly correlated with the severity of spinal cord injury (ASIA total score) ( r=0.668, P<0.001). Conclusions:In patients with acute cervical hyperextension injury and CCS, the MEP latency is prolonged, the amplitude lowered, and the motor threshold enhanced. The MEP latency is strongly correlated with the severity of spinal cord injury and upper limb motor function. The MEP-induced state is also closely related to the severity of spinal cord injury.

Objective:To study the clinical efficacy of Chaihu Shugansan on non-alcoholic fatty liver(NAFLD) patients with liver stagnation and spleen deficiency syndrome and its effect on intestinal flora. Method:The study was a single-center, randomized,single-blind, placebo-controlled clinical study involving 80 patients with NAFLD treated from January 2019 to January 2020 at our hospital. They were divided into two groups (Chaihu Shugansan group,n=40) and control group (placebo group,n=40). The two groups of patients were given lifestyle intervention as the basic protocol. The treatment group was orally given Chaihu Shugansan, and the control group was orally given placebo. The drugs were given twice in the morning and evening, 1 dose/time. Patients were followed up for 12 weeks. Before and after treatment, the efficady on liver steatosis was observed by abdominal ultrasound and transient elastography (Fibroscan), levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),glutamyl transpeptidase(γ-GT),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),total cholesterol(TC),triglyceride(TG),interleukin(IL)-6,IL-1β,Toll-like receptor-4(TLR-4) in peripheral blood mononuclear cells(PBMCs) and intestinal flora were also detected. Result:There were 37 patients in the treatment group and 35 patients in the control group who finally completed the study protocol. The total effective rate of NAFLD in the treatment group(81.08%,30/37) was higher than that in the control group (68.57%,24/35)(Z=2.67,P<0.05). The treatment group was superior to the control group in reducing the levels of BMI,ALT,AST,TC,LDL-C,TG,γ-GT and increasing the level of HDL-C(P<0.05). The levels of pro-inflammatory cytokines(TNF-α,IL-1β and IL-6),the values of Controlled Attenuation Parameter(CAP),Liver Stiffness Measurement(LSM) and expression of TLR4 were down-regulated in the treatment group (P<0.01). In addition, the treatment group showed increase in the abundance of beneficial bacteria (Bifidobacterium and Lactobacillus) and inhibited the abundance of pathogenic bacteria (Enterobacter and Enterococcus) in the gut(P<0.01). Conclusion:In addition to the lifestyle intervention,Chaihu Shugansan can improve lipid metabolism and liver function, regulate intestinal flora and inhibit the level of inflammatory factors in patients with NAFLD.

ABSTRACT

OBJECTIVE@#To evaluate the efficacy of electro-acupuncture (EA) or transcutaneous electrical acupoint stimulation (TEAS) on perioperative cerebral blood flow (CBF) and neurological function in ischemic stroke (IS) patients undergoing carotid artery stenting (CAS).@*METHODS@#In total, 124 consecutive IS patients were randomly allocated to the EA, TEAS, and sham groups (groups A, T, and S; 41, 42, and 41 cases, respectively) by software-derived random-number sequence. Groups A and T received EA and TEAS, respectively, at the Shuigou (GV 26) and Baihui (GV 20), Hegu (LI4) and Waiguan (TE 5) acupoints. Group S received sham EA. The stimulation was started from 30 min before surgery until the end of the operation. The primary outcome was the CBF at 30 min after surgery, which was measured by transcranial Doppler sonography. The secondary outcomes included hyperperfusion incidence and neurological function. National Institutes of Health Stroke Scale (NIHSS) and General Evaluation Scale (GES) scores were recorded at 1 week, 1 month, and 3 months postoperatively.@*RESULTS@#Mean CBF velocity at 30 min after surgery in groups A and T was much lower than that in Group S (P < 0.05); the incidence of hyperperfusion in Groups A and T was also lower than that in group S (P <0.05). Acupuncture was an independent factor associated with reduced incidence of hyperperfusion (OR=0.042; 95% CI: 0.002-0.785; =0.034). NIHSS and GES scores improved significantly at 1 week postoperatively in Groups A and T than in Group S (P < 0.05). Relative to Group S, groups A and T exhibited significantly lower incidences of moderate pain, as well as higher incidences of satisfaction with anesthesia, at 1 day postoperatively (P < 0.05).@*CONCLUSIONS@#EA or TEAS administered in combination with local anesthesia during CAS can inhibit transient increases in CBF, reduce the incidence of postoperative hyperperfusion, and improve neurological function. ABSTRACT

@#【Objective】To investigate the clinical value of chromosomal microarray analysis(CMA)for fetuses with persistent left superior vena cava(PLSVC).【Methods】Fetuses that were diagnosed with PLSVC during ultrasound examination and underwent invasive prenatal testing(on which karyotyping and CMA were both performed)from January 2014 to December 2016 at the First Affiliated Hospital of Sun Yat-sen University were reviewed. According to the combination of other ultrasound abnormalities, the cases were divided into isolated group and complicated group.【Results】Karyotype analysis identified chromosomal aberrations in 18.5%(15/81)of the fetuses, while CMA detected pathogenic copy number variations(CNV)in 23.5%(19/81)of the fetuses. There was no significant difference in the detection rate of chromosomal abnormalities between the Karyotype analysis and CMA(P = 0.44). CMA achieved an incremental yield of 6.1% (4/66)among PLSVC fetuses with normal karyotypes, and only in the complicated cases. There were 12 cases(14.8% ,12/81)in isolated group and 69 cases(85.2% ,69/81)in complicated group. The frequency of genetic anomalies in the complicated group was not significantly higher than that in the isolated group(26.1%,18/69 vs. 8.3%,1/12,P = 0.277). The incidences of atrioventricular septal defect, facial abnormalities, and multiple soft markers were significantly higher among fetuses with abnormal genetic test results(P= 0.030,P= 0.012,P= 0.014).【Conclusion】CMA is a valuable tool for identifying additional unbalanced submicroscopic chromosomal abnormalities in fetuses with PLSVC ,especially when PLSVC is accompanied by other ultrasound malformations.

ABSTRACT

@#【Objective】This study aimed to explore copy number variations(CNV)in fetuses with conotruncal heart defect (CTD). 【Methods】 Fetuses with ascertained CTD were investigated for chromosomal aberrations including copy number variations with chromosomal microarray analysis(CMA)and QF-PCR. Based on clinical significance of CNV,Fetuses were divided into two subgroups:non- benign CNV group [(pathogenic CNV and CNV of unknown significance(VOUS)]and benign CNV group. Data on fetal structural malformations, copy number variations, and pregnancy out? comes were collected and compared.【Results】Among 128 cases without chromosomal aneuploidies ,pathogenic CNV , CNV of VOUS ,and benign CNV were identified in 5.5% ,4.7% ,and 3.9% ,respectively. Compared with cases in benign CNV group(n=115),fetuses in non- benign CNV(n=13)had a significantly higher rate of overall extra- cardiac anomalies(76.9% vs. 43.5%,P=0.037),structural extra-cardiac anomalies(61.5% vs. 24.2%,P=0.022),softer mark? er anomalies(61.5% vs. 20.9% ,P=0.004),and thymus anomalies(30.8% vs. 0.87% ,P=0.000),whereas,no significant difference in that of intra- cardiovascular anomalies was noted(53.9% vs. 53.9%,P=1.000)excepted for that of persistent left superior vena cava(46.2% vs. 13.9% ,P=0.010). The incidence of natural death in non- benign group was higher than but not statistically different from that of benign group.【Conclusions】Pathogenic CNV contributed to the pathogenesis of CTD. The presence of associated extra-cardiac anomalies including thymus abnormalities correlated with a higher probability of non-benign CNV.

ABSTRACT

AIM: To investigate whether oxytocin has neuroprotective effects on hippocampal CA 1 pyramidal neurons from neonatal rats exposed to hypoxic-ischemic brain injury and the underlying mechanisms.METHODS:An in vitro model of hypoxic-ischemic injury was used by exposing the brain slices to oxygen-glucose deprivation(OGD)solution. Acute dissociated brain slices(6~8 slices per rat)from 8 Sprague-Dawely rats of 7~10 d old were used. The slices were randomly divided into 4 groups: control group, OGD 20 min group, OGD 40 min group and OGD +oxytocin group. The effect of oxytocin on neuronal death was evaluated by TO-PRO-3 staining.Fresh brain slices from other 20 neonatal rats were divided into OGD group,OGD+oxytocin group,OGD+dVOT(oxytocin receptor antagonist)+oxytocin group, and OGD+bicucuclline(GABAAreceptor antagonist)+oxytocin group. The onset of anoxic depolarization in the hippocampal neurons treated with different drugs was recorded by whole-cell patch-clamp techniques.RESULTS: The results of TO-PRO-3 staining showed that neuronal deaths in hippocampal CA 1 area were increased over the prolonged OGD time.Oxyto-cin significantly reduced the hypoxic-ischemic deaths.Oxytocin dramatically prolonged the onset time of anoxic depolariza-tion after the application of OGD solution. Both dVOT and bicuculline blocked this effect.CONCLUSION: Oxytocin plays a neuroprotective role in neonatal rat hippocampal CA 1 pyramidal neurons by enhancing the inhibitory synaptic trans-mission via oxytocin receptors. Therefore, oxytocin is useful as a candidate for neuroprotective treatment after neonatal hy -poxic-ischemic brain injury.

ABSTRACT

Background and purpose:miR-196a2 functions as an oncogene during tumor initiation and pro-gression. The up-regulation promotes tumor cell proliferation, invasion and metastasis. Therefore, it is promising to be an important tumor biomarker. The aim of this study was to investigate whether rs11614913, a gene polymorphic site ofmiR-196a2, is associated with the risk of leukemia. Methods:A case-control analysis was employed. Bone marrow or periph-eral blood was collected from 210 leukemia patients diagnosed from Jan. 2009 to Jul. 2015 in Yantaishan Hospital (case group) as well as 250 healthy people who were physically examined during the same period (control group). Polymerase chain reaction-restriction fragment length polymorphism (PCR-PFLP) was used to detect the genotype of rs11614913. Application test was used to compare the difference in the frequency of each genotype between case group and control group. The odds ratio (OR) of SNP allelic genes was calculated using logistic regression analysis and 95%CI represented the risk of leukemia for each genotype. Results:The distribution differences in the frequency of T/T, C/C, C/T genotype of miR-196a2 rs11614913 between case group and control group were statistically significant (P<0.05). The risk of leukemia for individuals who carried mutant homozygous C/C was 2.661-fold higher than those carried wild-type homozygous T/T, and the difference was statistically significant (P<0.05).Conclusion:ThemiR-196a2 gene polymorphic site rs11614913 was associated with the risk of leukemia. ABSTRACT

In our previous study, we have elucidated the chemical profile of YGS40, a fraction of Yi-Gan San (YGS), used for the treatment of Alzheimer's disease (AD). Oxidative stress-induced apoptosis is implicated in neurodegenerative disorders such as AD. The aim of the present study was to explore the protective effects of YGS40 against hydrogen peroxide (H2O2)-induced apoptosis in PC12 cells and the underlying mechanisms. PC12 cells were exposed to 100 μmol·L(-1) of H2O2 for 12 h with or without YGS40 pretreatment. Cytotoxicity was determined by MTT (3, (4, 5-dimethylthiazole-2-yl) 2, 5-diphenyl-tetrazolium bromide) and lactate dehydrogenase (LDH) release assays; apoptosis was detected by Annexin V/propidium iodide coupled staining and by determining caspase-3 activity and Bax and Bcl-2 protein levels. Mitochondrial membrane potential (MMP) was assessed by the retention of rhodamine123; and the activities of superoxide dismutase (SOD) and malondialdehyde (MDA) were measured using commercially available enzymatic kits. Pretreatment with YGS40 significantly prevented H2O2-induced cytotoxicity and protected the cells against H2O2-triggered apoptosis characterized by externalization of membrane phosphatidylserine and caspase-3 activation and the increased ratio of Bax/Bcl-2 in PC12 cells. Further studies showed that YGS40 suppressed H2O2-induced MMP loss, increased SOD activity, and decreased MDA level. These findings suggest that YGS40 may be beneficial for the prevention and treatment of oxidative stress-mediated disorders.
